Request for change of final exam

Please be aware that students may request changes in final exams only if:

1) The student has 3 or more final examinations scheduled on one day, or

2) The student has a work schedule which conflicts with an exam time, or

3) There is a personal emergency beyond the control of the student. In this case, may request a change immediately.

You may complete and submit the form online. Please complete the information for ALL of your exams, not just the one(s) you wish to change. For full consideration, please do so no later than 2 weeks prior to the first day of final examinations unless it is a personal emergency. In that case, you may request a change immediately. You may indicate a preferred time to take the exam, but it will depend upon the schedule of the instructor. You will receive confirmation of your request from the Vice President for Academic Affairs/Provost and/or your instructor.
