It's Know Your Numbers time: Oct. 30-Nov. 3

Heidelberg is excited to offer all benefits-eligible employees and their spouses/partners the opportunity to participate in a health risk assessment designed to provide valuable information to assist you in taking charge of your long-term health. The assessments are a collaborative effort between Mercy Health-Tiffin, Know your Number™ (KYN) and Heidelberg University.
The assessments will be held in Campus Center between 6:30 a.m. and 10:30 a.m. on Oct, 30-Nov. 3. The following screenings will be performed:
• Fasting glucose & lipid profile including cholesterol, triglycerides & ratios (blood tests)
• Measurement of blood pressure, height, weight and waist circumference
• Measurement of body fat composition and BMI
• Bone density screening (heel scan)
• Carbon monoxide screening
All participants MUST complete an online pre-screening questionnaire PRIOR to making their appointment. Beginning Monday, Oct. 9, employees can access the online questionnaire. The questionnaire will take less than 15 minutes to complete.
After completing the questionnaire, you may schedule your on-campus appointment. Information for scheduling your appointment, make-up dates and the 2018 Wellness Incentives is included in this document. Please do not hesitate to contact HR with any questions!
