Schedule Changes Form

Department Chairs/Directors are to complete the form to approve a schedule change once the schedule becomes official by the Provost/ACLT. Requests are sent to the Assistant Registrar. Final approval may be requested from the Provost. Department Chairs/Directors must review for any potential ripple effects/conflicts and should any appear, they must be rectified prior to submitting the form approving the requested change.

Faculty course load
Please also include how faculty load will be adjusted for course deletions, or when an over-load if it has been approved by the Provost.
