Undergraduate Change of Major / Minor / Concentration

This form is intended solely for use by Academic and Professional Advisors. Students should not complete or submit this form. It is used to add, remove, or change a student's major(s) or minor(s). Before submitting this form, advisors must have a conversation with the student regarding the proposed changes and how they may affect the student’s graduation timeline. 

Please note: If a student changes their major, a new academic advisor will be assigned automatically.
