Request for Change of Final Exam

Students: Please be reminded that the Office of Academic Affairs allows students to request changes in final exams only if:

• You have 3 or more final examinations scheduled on one day, or
• You have a work schedule which conflicts with an exam time, or
• You have a personal situation beyond your control

All requests for exam changes should come through the Office of Academic Affairs. Forms to submit Requests for Change of Final Exam are available here.

You may complete and submit the form online. Note that you are asked to provide information for ALL of your final exams, not just the one(s) you wish to change.

Please complete the form by Monday, April 22, 2019. Dr. Vicki Ohl will be in contact with your instructor and then with you once a decision and arrangements have been made.

If you do not receive a response within 5 days, please re-submit your request.
