Appointments are made through the Stoner Health Center at 419-448-2041. Facilities are closed Saturday / Sunday and during the summer session.

New Student Medical Records Portal

All new students must complete their medical forms through the electronic medical records portal. A full list of required immunizations/vaccinations is available on Inside.

*If you are a student athlete, you must fill out both athletic forms and new student health forms.

Eligibility

All students are eligible for care and treatment at the health center.

Services

  • Health and Wellness screenings
  • Minor Illnesses and Injuries
  • Mental Health Counseling
  • Lab testing
  • Physicals (school, sports, employment, etc.)
  • STI Screening

Medications

Over-the-counter medications may be charged to the student’s account. Students will be able to choose where to have prescriptions filled at a local pharmacy such as Kroger, Wal-Mart or CVS.

Required Immunizations

  • Diphtheria/tetanus booster–received within the past 10 years
  • Measles, mumps, and rubella–1 initial (early childhood) and 1 booster (childhood)
  • Oral polio–3-4 doses (childhood)
  • History of chickenpox disease or varicella vaccine
  • Hepatitis B vaccine–series of 3 doses (series can be completed at the health center)
  • The following immunizations are recommended:
    • Meningococcal vaccine, Influenza vaccine (annually), COVID-19 vaccine.
